Looks like you didn't run the SQL. Did you do the step

It was the second step in the readme, right after it says for you to upload the included files and right before you edit your first file.

What do you mean configure everything? Are you still getting the same error? Version 3 puts a little check box at the bottom of the reply/post vs the other versions being in a specific forum section, like I have it on my site.

that's because you need to set it up as your home module, then nothing will show until you may a post that you mark as "news".
To make the Forum News module your home module, you need to go into ACP -> Modules
( aka: YourSite.com/admin.php?op=modules ).
Once in there, scroll down until you see Forum_News. Once you see that, you will see starting on the far left two check options. The first column is to make it active, so check that if it isn't already. The next column is to make it the home module, Check that as well. Then go all the way to the right column, and select "Edit". Once in there, you will have more settings. If you want on your home page to have both left and right blocks show up, then you would change the visible blocks to "Both".
Note:
The "Who can see this" MUST be set to all visitors. That is also controlled when you click edit. If its not set to that and you don't see that option when you click edit, then make something else the home module temporarily, then change the "who can see this", then go back and make it the home module again.
